Publication of CYSUR 7 2018 - Concise Child Practice Review

The Mid and West Wales Safeguarding Board has today published the report of the Concise Child Practice Review, reference CYSUR 7 2018, in respect of a child who resided in Carmarthenshire. 

The Independent Review was commissioned by the Mid and West Wales Safeguarding Board and was led by Gladys Rhodes-White OBE. It was undertaken in accordance with statutory legislation set out in the Social Services and Well-being (Wales) Act 2014 and accompanying guidance Working Together to Safeguard People Volume 2 – Child Practice Reviews (Welsh Government, 2016). 

The Mid and West Wales Safeguarding Board will now oversee the delivery of an action plan and all agencies on the Board are committed to ensuring that the recommendations are implemented to support improvement in practice. The review has been an opportunity to reflect and share learning amongst all partner organisations and practitioners on a multi-agency basis, and we acknowledge the commitment and contribution of those who have taken part in the review process.

The Board would like to express its deepest sympathy to the family and thank them also for their willingness to meet with the reviewers and share their thoughts and memories of their cherished son.
