Publication of CYSUR 2 2018

Today the Mid and West Wales Regional Safeguarding Board publishes the report of the Extended Child Practice Review, CYSUR 2 2018, in respect of an 18 year old young person who resided in Ceredigion.

The Review has been undertaken in accordance with statutory legislation set out in the Social Services and Wellbeing (Wales) Act 2014 and accompanying guidance Working Together to Safeguard People – Volume 2 – Child Practice Reviews (Welsh Government, 2016).

Ceredigion County Council has made the following statement to accompany the publication of the report.

‘Ceredigion County Council conveys its sincere condolences to the young person’s family, carers, friends and all those who have been affected by this tragic death.

The commitment and contribution of all who have taken part in the review process during this difficult time has ensured that the report provides a clear picture of the young person’s life and the challenges they faced.

The review has been an opportunity to reflect and share learning amongst all partner organisations and practitioners on a multi- agency basis.

We hope that the report will contribute to wider ongoing learning in relation to a number of key safeguarding issues.’

The Mid and West Wales Regional Safeguarding Board will oversee the delivery of an action plan and are committed to ensuring that lessons continue to be learnt and services improved.
